Before scheduling your nipple tattooing, you'll need clearance from your plastic surgeon. Most surgeons recommend waiting at least 3–4 months after your most recent breast surgery to ensure proper healing.
If you've undergone radiation therapy, you may need to wait longer to allow the tissue to fully heal and stabilize. Every body is different, so we’ll help you coordinate the best timing based on your unique treatment and recovery.

Most clients experience very little pain during nipple tattooing, largely because the reconstructed area typically has reduced sensitivity. You may feel some light pressure or vibration throughout the session, but it’s usually very tolerable.
If you do have more sensation in your reconstructed breast, we can apply a topical numbing solution to keep you comfortable.

When performed in a professional setting, nipple tattooing is generally very safe, with minimal risks such as infection, allergic reaction, fading, or raised scarring.

The Women’s Health and Cancer Rights Act of 1998 requires insurance companies to cover breast reconstruction after mastectomy — and that includes nipple-areola tattooing.
At Perky, we’re happy to provide all the documentation you need to file for insurance reimbursement. While we don’t bill insurance directly and cannot guarantee reimbursement, we make the process as smooth as possible for you.

Surgeon Clearance & First Tattoo Session
Once your plastic surgeon has given clearance and you feel ready, we’ll schedule your first tattoo session. During this appointment, you’ll be involved in mapping the placement and selecting colors. We’ll guide you through all your options and ensure you’re comfortable before starting.Healing Timeline
Initial healing takes about 10–14 days. The full color results usually become visible around 6 weeks after your first session.Touch-Up Session
A touch-up is scheduled roughly 6 weeks after your first session to perfect the color and details. If you’re traveling from out of town, we’re happy to adjust this timing for your convenience.Final Check-In
About 6 weeks after your touch-up, we’ll check in with you—either in person or virtually—to ensure you’re happy with your results and address any questions.

Session length varies based on factors like whether you’re getting one or both nipples tattooed, the size of your areolas, and the condition of your skin after surgery. Here are typical appointment times:
One-sided nipple tattoo:
First session: 1.5 to 2 hours
Touch-up session: 1 to 1.5 hoursBilateral (both sides) nipple tattoos:
First session: 2 to 2.5 hours
Touch-up session: 1.5 to 2 hours

The touch-up is a crucial part of your tattoo journey. It allows us to refine any areas that didn’t heal perfectly—especially around scar tissue—and boost the color’s vibrancy and longevity. This session also gives you the chance to adjust the color, size, or fine details so your nipple tattoo looks as natural and beautiful as possible.

We take great care with your skin during tattooing. The needle only penetrates the top layers of skin and does not reach implants or sensitive tissues. When performed by trained professionals, medical tattooing is a safe procedure.

We use traditional tattoo inks that are permanent; however, some fading over the years is normal due to sun exposure and natural skin aging. While your tattoo won’t completely disappear, you might choose to get a touch-up every 5–10 years to refresh its vibrancy—though this is entirely up to you.

Nipple reconstruction is a procedure your surgeon can perform after a mastectomy to create a small nipple mound using your own skin. This gives the reconstructed breast a small projection, mimicking the shape of a natural nipple. However, it doesn’t restore the coloring of the areola.
Once the area has fully healed, tattooing can be done to add realistic color and detail. It’s important to know that over time, the reconstructed nipple may flatten or lose some of its projection — which is why some people choose 3D nipple tattoos alone, or in combination with reconstruction, to achieve the look they want. Some individuals are not candidates for nipple reconstruction. Talk with your surgeon to determine if nipple reconstruction is right for you.
3D nipple tattooing is a non-surgical option where a highly skilled artist uses advanced shading and highlighting techniques to create the illusion of a raised nipple. The skin itself stays flat, but the result looks incredibly realistic — even up close.
Many people choose this option for its natural appearance without the added projection of surgical reconstruction. Some also prefer it because it avoids the constant “headlight” effect that can come with a physical nipple mound.

Tattooing over scar tissue requires special care because the skin is more delicate and unpredictable. That’s why it’s essential to work with an artist experienced in medical tattooing and scar work.
Scar tissue can vary in how well it absorbs pigment, which means you may need additional sessions to achieve your desired results. With the right approach, though, it's absolutely possible to get beautiful, natural-looking results on or around scars.
In some cases, you may be advised to massage your scar tissue in the weeks leading up to your appointment. This can help soften the area and improve ink absorption, setting the stage for the best possible outcome.

If you have photos from before your surgery, they can be a helpful reference when planning your nipple tattoos. These images allow us to assess the natural tone, size, shape, and placement of your original nipples.
That said, many of our clients don’t remember exactly what their nipples looked like — and that’s completely okay. You’re not required to replicate your previous appearance. In fact, this can be an opportunity to make small changes if there were aspects you didn’t love before. The goal is to create something that feels right for you now.
