Perky is here because we understand there is power in reclaiming and reimagining your nipples, or choosing to not have them altogether. Nipple tattoos aren't one-size-fits-all. There are so many beautiful, personal ways to make yours feel uniquely you, whether you're honoring what was or embracing something entirely new.

If you choose nipple restoration, it is typically the “cherry on top” — or final step — in your breast reconstruction process, adding the final touches to your overall result. But, did you know there are different types? There are 3 options when it comes to nipple reconstruction:

What is nipple restoration?

C-V Flap

Nipple Graft

3D Nipple

So, one of the big decisions you have to make is — Do I want nipple reconstruction or not?”

There are pros and cons to surgical nipple reconstruction, like- “Do I want my headlights on 24/7?” Also, keep in mind: not everyone is a candidate for nipple reconstruction. What you are offered depends on the type of breast reconstruction you’ve had, your skin quality (thickness, presence of radiation damage, scarring) and your surgeon’s preferred approach. Ultimately it's a very personal decision and a great topic to chat through with your surgeon.

Picking Size, Colors, & Placement

There are many different ways to approach your nipples, but ultimately youre the boss! You’re involved in every step — from selecting the colors, size, and shape, to the finer details of the design. Our goal is to create nipple tattoos that reflect your aesthetic goals.

Whether you choose Perky or another artist, it’s essential that you're part of the planning process. Sadly, we’ve heard stories where clients were placed in the chair with little to no input. This is your body, and you deserve to have a voice in how your tattoo session goes. Always feel empowered to advocate for yourself.

Who Can Get Nipple Tattoos?

Every scar pattern is unique, but whether you've had implant-based reconstruction, a flap procedure, or chosen to stay flat, you're likely a great candidate for 3D nipple tattooing.

Here are examples of nipple tattoos from a variety of reconstruction types and scar patterns we’ve worked with:

Bilateral Implants + C-V Flaps

Bilateral DIEP + 3D Nipples

Bilateral DIEP + 3D Nipples

Aesthetic Flat Closure + 3D Nipples (Fresh)

Unilateral C-V Flap (Matching Native Left Nipple)

Bilateral Implants + 3D Heart Nipples

Bilateral DIEP + 3D Nipples

Bilateral Implants + C-V Flap

Bilateral DIEP + 3D Nipple Tattoos

Bilateral DIEP + 3D Nipples

Bilateral DIEP + C-V Flaps

Unilateral DIEP + 3D Nipple (Matching Native Left Side)

Fresh vs. Healed

It’s important to keep in mind that when you first get your tattoo, the colors will appear darker and more vibrant than they will after healing. The initial irritation will subside within about a week, and over the next six weeks, the colors will gradually soften and fade as the tattoo heals.

The photos on the left are fresh tattoos, while the photos on the right are fully healed results. As you can see, the appearance of your tattoos will change once it has healed. We always recommend asking any tattoo artist you’re considering for photos of their healed work, not just their fresh tattoos, so you get a true sense of their aesthetic and ensure that you’re happy with the end result.

How Many Sessions Will I Need?

Nipple restoration is typically completed in two sessions. The first session can take place once you are both medically and emotionally ready. The second session — the touch-up —is usually scheduled about six weeks later for our local clients.

For those who are traveling, a longer gap between sessions is absolutely fine and won’t affect your final results. We’re happy to accommodate your schedule to ensure the best possible outcome.

The touch-up session is a crucial part of the process. It's our chance to address any areas that may not have healed as expected, particularly around scarring, and to enhance the vibrancy and longevity of your tattoo. Additionally, this session provides the perfect opportunity to make any adjustments to the color, size, or finer details, ensuring your restoration looks as natural and beautiful as possible.
